NCHPAD is establishing sustainable approaches to health promotion and wellness with people with disability that can be scaled across healthcare facilities and into communities across the United States.
Our Data Coordination Center (DCC) is working to integrate people who have recently accessed the healthcare system with a new or existing disability into home (tele wellness) and community-based programs and services to promote their health and wellness.
We are building an expansive network of healthcare providers who can refer people with disabilities to NCHPAD to further their health, wellness, and community inclusion. Our long-term goal is to empower and work with people with disabilities to participate in self-managed and co-managed, with a health coach, health and wellness programs in their own communities. We aim to build linkages with people with disabilities to have greater access to, and inclusion in, home (tele-wellness) and community wellness programs and public health initiatives that promote wellness in areas such as exercise, nutrition, and mindfulness.
